ABOUT THE FOUNDATION

The National Minority Health Month Foundation has developed the most comprehensive data platform for identifying and isolating the prevalence of chronic illnesses by zip code at the national, state, and community level. This unique convergence of information is used to identify “health disparity zones” which can assists healthcare providers, administrators, community-based and faith-based organizations, and policymakers to better deliver resources to communities that have priority needs.

The Foundation was launched in April 2001 in response to and in support of Healthy People 2010 . During the 107 th Congress, the United States Senate and House of Representatives passed a resolution that calls upon the President to proclaim the month of April as National Minority Health Month.

The Foundation seeks to quantify the disproportionate burden of healthcare disparities and implement solutions through wide reaching collaborative efforts.

Foundation staff provide analysis and advocacy, including community-based strategic planning, implementing targeted initiatives and programs, convening stakeholder networks, and conducting strategic national and local issue-based campaigns.

The Foundation is a 501(c)(3) organization that promotes the b uilding of coalitions and evidenced-based solutions towards the elimination of health disparities. Gary A. Puckrein, Ph.D. is Executive Director.
